According to the AAOS Musculoskeletal Coding Guide 2010, you score one bullet for each numbered element described in the HPI.
Example: Does this mean if you have more than one pain location (i.e., low back, knee, ankle, shoulder) described, you count as one point under Location?
Example: Does this mean if you have several "what makes better and what makes worse" do you count them all as one point under Modifying Factors?
In other words, if you have more than one factor fulfilling an element of the HPI, (i.e., the elements are location, quality, severity, duration, timing, context, modifying factors, and associated symptoms), do you count as one point?
